here will be an important tool for your key staff as they try to navigate the roll-out of the CCC+ Program which will be the managed care system for acute and primary care for all of the individuals that we support. The handout gives critical information about enrollment, what you as a residential provider need to do now and how you can maximize the benefits!
This can and will be a win-win for everyone – all of the Health Plans participating in CCC+ have included an adult dental benefit! See the
comparison chart to determine the differences among the plans. Another key element of the requirements for the Health Plans is to aid and facilitate transitions, for example, the transition from an acute care setting back “home!.”
