In addition to releasing the checklist that all Waiver Providers will be required to complete and retain in their records until March 17, 2019 (the checklist can be found at http://www.dbhds.virginia.gov/professionals-and-service-provider/developmental-disability-services-for-providers) DBHDS has also shared the text of the Survey Monkey Tool that will be completed to summarize the provider’s checklist results. You can use the survey text to help you craft answers to the checklist.
