DMAS Enrollment – How to Answer Question #17

For not-for-profit providers, the DMAS enrollment forms can be a bit of a challenge.  DMAS is required to document management and/or ownership interests as part of their reporting for the APA.  If you feel that your Board of Directors would not want to share their dates of birth or Social Security numbers; you may choose to use the details for your CEO as the “managing employee” in answering Question #17.  You simply must complete all required fields; as there is no “ownership” interest, the field for “percentage of ownership” will be blank.
