New Option for COVID Vaccines for Residential Programs

A VDH Long-Term Care Facility Vaccination Coordination Team has been established under the VDH State Pharmacy to coordinate the current and on-going need for vaccinations at long-term care facilities, including skilled nursing facilities, assisted living facilities, independent living facilities, group homes, and other residential care settings.

An intake process has been established in REDCap for facilities to report their vaccination needs.  The link to that form is https://redcap.link/VaccineLTCFs.Virginia.gov.  This form should be completed by a designated Point of Contact at the LTCF (not by individual residents).  Lastly, if an LTCF has an upcoming vaccination clinic already scheduled, they do not need to complete this survey.
