VNPP has submitted two Issue Briefs which describe innovative integrated care for individuals with ID/D. The goals of the proposals are to improve the individual’s experience and medical outcomes, reduce unnecessary visits to the Emergency Room, shorten the length of hospital stays and improve the quality of the transitional care pre and post discharge.
